To request a foreign credential evaluation, please download one of our Credential Evaluation Applications. Please note that there are three different applications (one for all course-by-course evaluations, one for law firms and businesses, and one for individuals). Each is available in either Adobe Acrobat format or as a Microsoft Word Template.

If you plan to complete the application form by hand, please use Acrobat format. Application forms that are completed by hand may be submitted by faxing them to (425) 248-2262.

If you want to complete your application form electronically, use the Microsoft Word document template. Forms completed using this template may be submitted via e-mail to info@fis-web.com or via fax at (425) 248-2262.
