Following are the various FIS service applications. Please select the application form that is most appropriate for your service needs. Each application includes the prices, processing times, special handling, and other important service instructions and terms and conditions.
CREDENTIAL EVALUATION APPLICATIONS
To request a foreign credential evaluation, please download the Credential Evaluation Applications appropriate for your purpose. Please note that there are three different applications (one for all course-by-course evaluations, one for immigration law firms and businesses, and one for individuals). Each is available in either Adobe Acrobat format or as a Microsoft Word Template.
If you plan to manually complete the application form, you can use either the Microsoft Word or Adobe Acrobat version. If you want to complete your application form electronically, use either the Microsoft Word document template or the Adobe Acrobat form fill-able version. Completed application forms may be submitted via regular or express service mail, e-mail to info@fis-web.com or via fax at (425) 248-2262.

TRANSLATION SERVICES APPLICATION
To request translation services, please download our Translation Application in Adobe Acrobat format. If you need to request Translation Services in combination with Credential Evaluation services, please use one of our Credential Evaluation Applications above.

EXPERT OPINION SERVICES APPLICATION
To request Expert Opinion services, please download our Expert Opinion Application in Adobe Acrobat format and select the specific service that you require in the letter. You may wish to review the Expert Opinion FAQ to assist you in the selection of the service needed. An FIS foreign educational evaluation will also be provided for any academic credential(s) earned, when an educational + work experience evaluation equivalency is requested.
